The Federal Capital Territory Administration (FCTA) has inaugurated a committee to investigate the circumstances leading to the death of Miss Kafayat Abdul-Azeez at Asokoro Hospital.

The six-member committee headed by Dr. Evans Onyekele  Director, Medical and Diagnositics of the Health and Human Services Secretariat (HHSS) is to among other things thoroughly investigate the detailed management of the deceased and determine its adequacy or otherwise.

Also, the committee is mandated to determine any evidence of negligence in her management on the part of any member (s) of the team that managed her, as well as recommend appropriate sanctions where necessary on any staff found negligent in her care.

According to the HHSS Director of Adminstration and Finance (DAF), Alh. Abdulraheem A. Musa who formally inaugurated the committee, on behalf of the HHSS Secretary, Mr Adamu Bappah, said members of the committee were chosen based on experience and expertise and have two weeks to submit their report.

Mr Bappah added that the committee is to recommend necessary measures to address any significant fallout from the investigation to forestall similar occurrences in the future and make any recommendations, which in the view of the committee would enhance the quality of care provided in FCT hospitals.

While responding, Chairman of the Committee, Dr Evans Onyekele thanked the Secretariat, for deeming it necessary to investigate the unfortunate incident.

He assured the Administration of the member’s commitment to carry out its terms of reference within the stipulated two weeks timeframe.
